Objectives
The objectives of this study are to evaluate economic, clinical and humanistic outcomes in patients with either asthma, diabetes or osteoporosis-- through disease state management DSM programs conducted at community pharmacies. Data will be collected from several pharmacies belonging to one chain in the Southern California region. The experimental group will consist of pharmacies with newly implemented DSM programs and will be compared to control pharmacies from the same chain that do not have such programs. Pharmacists at each experimental site will be providing patient education and monitoring-- and interventiondrug therapy management as required. Comparisons between experimental and control groups will be used to evaluate the effectiveness of the pharmacist-directed DSM programs.
