Objectives
The objectives for this program include: 1 Developing a MTM Services Practice Development Online Course and Toolkit assisting pharmacies and pharmacists to: a Link to core and elective educational programs that grow patient care practice knowledge-- b Develop a pharmacy practice plan and framework for the delivery of services-- c Develop a sustainable business model for providing MTM services-- d Implement a continuous professional development plan for practitioners-- e Evaluate their practice and implement continuous quality improvement measures. 2 Identifying three pharmacies in rural Minnesota communities which are interested in introducing MTM services into their practices and assist these sites with implementing these services within one year. 3 Developing a plan for evaluating, updating and dissemination of the MTM Services Practice Development Online Course and Toolkit
