Help Your Heart is a patient care program that indentifies patients in the community with cardiovascular risk. The objectives for this program include the following: 1. Identification of patients in the community through screening. 2. Perform blood pressure and complete lipid panel on high risk patients. 3. Report results to patients primary care physician 4. Provide patient with education on medicatin compliance, life style improvement, and diet choices. 5. Follow up with patient in 6 month and repeat blood pressure and lipid tests.
