Objectives
The primary objective of this project is to develop an enhanced MTM focused clinical externship program. The addition of patient clinical care services and health education would : enhance the MTM services, create a practice and speaking venue for students from many healthcare disciplines, provide an invaluable service to our community, and forge partnerships between our community pharmacy and healthcare providers. Health education could be provided in the areas of : medication management Rx and OTC, asthma management, diabetes self-care management, smoking cessation, cardiovascular care, weight management, preventive medicine and other areas designed to lower healthcare costs and improve health outcomes and quality of life in our community. Having a pharmacy in a grocery store setting affords many opportunities to do MTM activities and discuss therapeutic lifestyle changes. Although the initial focus would be on developing the MTM focused clinical externship, it is our plan to expand by adding a community pharmacy residency next year strengthening ties with the Schools of Pharmacy and allow students and pharmacists to challenge themselves and expand their knowledge and patient care skills along the guidelines of Project Destiny. We plan to share our experiences through presentations and publications. Community pharmacists do make a difference, which we are ready to demonstrate by taking MTM to the next level as Patient Care Pharmacists.
