Objectives
A pharmacy that provides coverage for Medication Therapy Management MTM services shall include processes to improve continuity of care, outcomes, and outcome measures. Pharmacist need to develop ways to collect, evaluate, and measure MTM outcomes. Imetrikus, is a health care technology company in the business of providing a modern, interactive connection between those who are chronically ill and the health professionals who care for them. Imetrikus has developed an advanced population health, chronic care management application and service called Medicompass. iMetrikus has products and services that will enable pharmacists to upload healthcare data from patients devices such as blood glucose meters and blood pressure monitors and an interactive web-based system which allows pharmacists to create their own patient registry to help patients comply with their treatment regimens. Proposal To evaluate Medicompass as a MTM outcome collection software in the community pharmacy setting at nine North Carolina Kerr Drug Pharmacies, including Zebulon, Chapel Hill, Benson, Raleigh-Lassiter Mill, Raleigh-Towne North, Raleigh-South Hills, Lenoir, Asheville, and Fuquay-Varina.
