Objectives
Causeys pharmacy, an independent community pharmacy, has developed a program designed at reducing the cost of self-insured prescription plans for employers of 200-1200 employees by approximately 12%. The program is designed for a clinical pharmacist to go onsite twice per week and implement medication therapy management to working employees. The clinical pharmacist will work with the employer, employees, PDP, and physicians to assist with utilizing formulary preferred agents, point of care testing, implementing disease state management services, designing drug treatment therapies, and education programs. a. The primary objective will be to develop an onsite medication therapy management clinic at an employers worksite. b. Secondarily, train and prepare a pharmacy practice resident to be able to assume the responsibilities of development, implementation, ownership, clinical confidence to develop a medication therapy management clinic in the community pharmacy setting.
