Objectives
Pilot projects with local physicians practices of a pharmacist meeting with patients once or twice a month. Program will prove to Physicians that this will not only greatly improve patient health but will increase their incentives from the PGIPs and other programs. I want to prove that a pharmacist is necessary for this role, nurses or others will not have the same results.
