Objectives
The Alliance for Integrated Medication Management Collaborative AIMMc is a national effort to coordinate health care by integrating team-based medication management services into the care of high-risk, high-cost patients suffering from multiple chronic health conditions. In order to model and evaluate the impact of collaborative relationships between community pharmacy and primary care practitioners, we proposes to create a cohort of 7-12 AIMMc teams focused on delivering integrated medication management services and are committed to sharing the results of their work with a national audience. Organizations enrolled in this cohort would meet defined criteria for integrating the role of community pharmacists in to team-based care, agree to track outcomes of patients served within a defined population of focus and commit to sharing results with AIMMc teams and external stakeholders via publications and presentations.
