Objectives
The IMPACT Care Transformation Institute will be a programmatic expansion of the Project IMPACT: Diabetes model into primary care through a combined offering of: web-based, modularized training, high-impact resources, and personalized implementation coaching and accountability. The IMPACT Care Transformation Institute will launch with a focus on empowering implementation of team-based care that improves patient outcomes and medication adherence. The audience for the IMPACT Institute includes providers such as pharmacists, physicians, nurse practitioners and physician assistants. APhA Foundation will provide website and call-line support to customize the training and implementation solutions to each interested provider. The Foundation will partner with APhA, AADE, Vital Simulations, IQWare, and others to aggregate training and implementation resources, including the Patient Self-Management Credential PSMC, AADE7 Self-Care Behaviors, APhAs Certificate Training Programs, SiMCare for Diabetes, the IMPACT Data Management System, and many other educational and care delivery resources.
