Objectives
This project will identify and highlight community pharmacypharmacist practices that have successfully integrated patient-centered comprehensive medication management CMM services in team-based collaborative care. We will highlight the impact on patient outcomes and the key attributes that allowed for community pharmacypharmacist to integrate medication therapy management services at the level of CMM as defined by the Patient-Centered Primary Care Collaborative into a viable and sustainable business model. No market scan and in-depth follow-up has focused exclusively on identifying a collection of expert and emerging community pharmacypharmacist practices that have successfully implemented this level of service and then explored factors necessary for success. Utilizing Health2 Resources project management expertise and extensive database and access to the Community Pharmacy Foundation Board contacts, we will devise and conduct a market scan to identify up to five expert and 10 emerging practices where community pharmaciespharmacists best represent a CMM level of service and integration into ACOpatient-centered medical homes or other coordinated care settings, and consider their overall impact. We will identify and share successful real-world practices from these organizations including tactics and strategies that can be replicated by other community pharmacypharmacist.
