AcademicAmbulatory care pharmacist partner with an Independent Pharmacy services and Primary care office. With the growing shortage of primary care providers, this model will test if clinical pharmacy services relieve this burden and allow clinicians to schedule more patients. Provider satisfaction levels will be measured. Goal is to implement a clinical program link between independent pharmacy, a primary care office and to test if the partnership allows for relief in patient scheduling. Objectives include providing clinical services in a retail pharmacy, and in the clinic, review medical charts, perform medication therapy management MTM, comprehensive medication review CMR, and Comprehensive medication management CMM. Further exploration includes investigating methods to increase key Star Ratings by improving quality measures. Bill for incident to services CMM, Wellness visits transitions of care TCM, Community based MTM, on appropriate platforms, as appropriate, to generate further revenue and show sustainability.
