CPF FUNDED GRANT DETAILS

Establish a pharmacist led, physician based, clinic to provide Annual Wellness Visits to the existing Medicare patients at two physician clinics. This physician-pharmacist collaboration will expand the community pharmacists role in providing clinical services to patients and lead to further collaboration between the professions. A training model developed from establishing this service will provide the framework for pharmacists to successfully execute this program within other physician clinics.
