Objectives
The goal of this project is to create replicable and transferable documents for the business model and data collection policies within an Ohio collaborative practice agreement between a community pharmacy and an outpatient clinic. Objectives are to: 1.Utilize the National Alliance of State Pharmacy Associations collaborative practice agreement report to create a deliverable document carrying out best practice policy and procedure for the initiation of an Ohio-based collaborative practice agreement2.Create a deliverable training protocol for implementation of an Ohio-based collaborative practice agreement for community pharmacy staff as well as outpatient clinic staff 3.Explore integration of Outcomes MTM data gathered in a community pharmacy with Epic data gathered in an outpatient clinic for synced patient documentation.
