Objectives

This grant would work to develop a medication reconciliation process that is driven by a pharmacy technician but incorporates questions that could improve OASIS measures. The targeted population would be high risk to very high risk patients determined by the home health agencys analyzer. The workflow that is developed will have a pharmacist evaluate the information gathered and communicate findings to the home health nurse. Readmission rates under this program would be evaluated at the conclusion and a payment structure would be established.
