Objectives
The primary impediment to community pharmacies being able to successfully document their care activities and be paid for the value that care creates is that current documentation methods are outside the normal workflow of a pharmacy. The Pharmacist eCare Plan is progressing in development to be a game changer to improve this situation dramatically, and does so using the established standards used by other health care providers. The type of data generated by the Pharmacist eCare Plan is critical for payers to be willing to pay community pharmacies for the positive outcomes they generate and to integrate community pharmacy with pharmacists in other care settings as well as other health care professionals. Deliverables from this work will be a white paper on the pharmacy workflow improvements from implementing the Pharmacist eCare Plan, an analysis of clinical data produced by community pharmacies using the eCare Plan, an eCare Plan website to host technical and promotional content, 15 vendors transacting eCare Plans and at least two payers using eCare Plans for documentation and payment to community pharmacies.
