Objectives
To pilot test a program that used a coach to assist pharmacies with practice transformation for the following: o Testing the initial change package focused on care planning and follow up activities o Applying the initial standardized change package and program for the pharmacies to follow throughout the practice transformation period o Determining the training, time, activities and touch points necessary to deploy a successful practice transformation coach o Testing data collection and reporting for practice transformation feedback using care plans submitted using the HL7 eCarePlan standard

Ongoing Impact
9/25/2019 - CPF Funded Study Piloted the Approach and Content to Assist Creating the Flip the Pharmacy Program.
Randy McDonough and Team Members conducted a rapid-pace pharmacy transformation program with select CPESN pharmacies in Tennessee and a local ACO during June - July 2019. They tested the utilization of a 'change package' structured curriculum and used a transformation coach who visited the pharmacies to provide timely hands-on guidance and monitoring. The total quantity and the quality of medication therapy problems, interventions and patient-centered goals in the pharmacy submitted patient care plans increased and improved significantly during the study period.
The learnings from the pilot study are shaping the content and approach of the Flip the Pharmacy (FtP) program. Follow the FtP Movement by signing up for the newsletter and downloading the monthly updates to the Change Package materials that will be developed for the six transformation domains.
