Objectives

Obstacles to effective medication management are complex. Vulnerable patients often have difficulty managing the medications they need due to a variety of barriers. Together, community pharmacies and community-based organizations (CBOs) are well-positioned to assist by addressing medication management issues related to health literacy and other social determinants of health (SDoH) barriers including: limited health care access, lack of care coordination between provider systems, and medication nonadherence. Primary objectives during the initial phase of this two-state initiative include: 1. Establishing collaboration between 2 community pharmacies and 2 CBOs in target communities with vulnerable patient populations to actively address communication and social determinants of health barriers to medication access and adherence. 2. Developing a communication tool to be used by pharmacists and CBOs to standardize communications, assess for SDoH barriers and facilitate patient access to medication therapy management (MTM) services and improved care coordination. 3. Developing a best practice implementation package and/or dissemination plan to be used by pharmacies and CBOs as a key component of the Flip the Pharmacy Behavioral Health/SDoH change package.
