Synopsis
Objectives

We propose establishing an ACT Pharmacy Collaborative Flip the Pharmacy Evaluation Team to report the collective care provided by pharmacists nationwide. We would engage the ACT Pharmacy Collaborative faculty leaders over the course of one year to meet a series of reporting milestones and to build a framework to guide future evaluation of community pharmacy-based care. There would be three components of this initiative: 1) Describe the work of the pharmacists utilizing eCare Plan available data (FtP year 1), 2) Pharmacist/Network Facilitator key informant interviews and 3) Create a framework to support broad use and assessment of eCare plan data nationwide.
