The Community Pharmacy Foundation and CPESN® USA Announce a 5-year Partnership to “Flip the Pharmacy” and Transform Community-based Pharmacy Practice
June 27, 2019 - The Community Pharmacy Foundation (CPF) and CPESN® USA are aiming to transform community-based pharmacies through a new dynamic program, “Flip the Pharmacy.” The “flip” comes from a movement away from point-in-time, prescription-level care processes and business models to longitudinal and patient-level care processes and business models. The current environment is ripe for a nationwide practice transformation effort to sustain community-based pharmacy practice through the creation of economically viable, scalable and sustainable care and business processes among clinically integrated networks who can contract with payers, purchasers and partners for high value, reliable and repeatable services across thousands of pharmacies. Flip the Pharmacy will more rapidly advance the opportunities presented by an emphasis on value-based care quality payments, clinical measures, and electronic care plans.
The development of CPESN® USA and their growing number of local, clinically integrated pharmacy networks across the country presents a geographically diverse cohort of community-based pharmacies eager to focus on quality patient care. “Pharmacy has generated an abundance of innovation over the past twenty years, but we’ve lacked the wherewithal to scale that innovation.” Remarked Troy Trygstad, Executive Director of CPESN USA. “We now have a twenty-year runway of opportunity following the publication of the Asheville Project, but no scalable and market-based value proposition to an increasingly consolidated health care system that is screaming for clinical outcomes that are usually dependent on optimizing medication use.”
Flip the Pharmacy will award qualified Practice Transformation Teams with funding and resources to act as implementation arms for locally-based community pharmacy practice transformation efforts. Six transformation domains have been identified as key components of the transformation efforts and implementation will be supported at the pharmacy-level by transformation coaches. Funding priorities include established payer relationships and lean budgeting with consideration given to geographic and population density factors. Over the 5-year partnership, the Program aims to graduate more than 1,000 pharmacies from a two-year transformation process modeled after similar Centers for Medicare and Medicaid Innovation efforts in primary care practices across the country. Additional program goals include lofty targets for non-product-based reimbursement revenue, care plan submissions, screenings for behavioral health conditions, reductions in systolic blood pressure, HbA1c percentages, and cholesterol in patients with associated chronic conditions as well as the completion of social determinants of health screenings.

The Community Pharmacy Foundation has a long and successful history of funding innovation efforts to advance community pharmacy practice and patient care delivery. The Foundation will introduce a new category of grant funding to their existing funding portfolio with this award of over $3.3 million to transform teams in the first two years of the program. Flip the Pharmacy augments the footprint and influence of CPF on the sustainability of community-based pharmacies by partnering with CPESN® USA to add an implementation component to its grant funding activities.
