Objectives
The Geriatric Pharmacotherapy Program at Virginia Commonwealth University plans to establish a model of pharmacy practice specializing in the care of elderly people. The practice model will be established at Plaza Professional Pharmacy, in Richmond Virginia. This pharmacy serves the needs of elderly people who are living independently as well as a large population of elderly in assisted living facilities. We will develop a variety of fee-for-service medication management programs and collaborative practices. A pharmacy resident, under the direction of pharmacists knowledgeable in geriatrics, will implement many of these programs and collect outcomes data from our interventions. The planned interventions are aimed at detecting drug related problems commonly seen in elderly patients and working with patients, their family, and their physicians to correct these problems. The overall objective is to improve outcomes from drug therapy in older people.
