Get The Medications Right (GTMR) & Joint Commission of Pharmacy Practitioners (JCPP) Meeting
Terry McInnis, MD, MPH, CPE, FACOEM (Blue Thorn, Inc.) shared highlights of the Get The Medications Right report with a group of pharmacy profession leaders at the November, 29, 2016 JCPP Meeting in Arlington, VA.
Referencing a slide that was adapted from Daniel Buffington, PharmD, MBA, (Clinical Pharmacology Services-CPS), Dr. McInnis began the presentation describing pharmacists' value in the progression of clinical pharmacy service intensity. The coordinated medication management continuum ranges from low to high depending on the complexity of specific patient needs. Dr. McInnis reiterated that pharmacists need to be the medication experts; there is demonstrated ROI and payers should incorporate and compensate.
The Get The Medications Right report is a nationwide snapshot of expert practices and clinical pharmacists delivering comprehensive medication management (CMM) in ambulatory/community pharmacy settings.

NCPA's Blog - The Dose: Independent Pharmacists Can Be Key Allies in Reducing Hospital Admissions – Earlier this month, the NCPA Blog, 'The Dose' highlighted a recent report by the Department of Health and Human Services (HHS) entitled "Rural Hospital Participation and Performance in Value-Based Purchasing and Other Delivery System Reform Initiatives." It revealed that hospitals participating in value-based payment programs struggle on readmissions metrics defined in the Hospital Readmission Reduction Program (HRRP). In fact, the report found fact 79% of rural and 76% of urban hospitals were penalized based on these measures.
The blog contains several references to value based payments, medication therapy management, transitions of care and pharmacist impact.
CPF has a special topic area for funded grants relating to community pharmacy and care transitions. There are several grants in varying stages ranging from in-study to completed. CPF funding of several grants in this area indicates the topic is of interest based on the potential for pharmacists to demonstrate value. Applicants interested in this topic area should review the funded grants and determine how a new grant would expand or enhance what has currently been funded. CPF is interested in grants that highlight new and emerging patient care innovations that are sustainable, transferable, and replicable in community pharmacy practice. Collaborations with payers, healthcare insurers and real-time data integration are of particular interest.

The second manuscript for this project was accepted for publication and is available as an open access, free download from the Journal of the American Pharmacists Association (JAPhA). This article is titled 'Pediatric medication use experiences and patient counseling in community pharmacies: Perspectives of children and parents.' The study aimed to explore the perspectives of children and parents regarding: 1) pediatric patients' knowledge and medication use experiences for chronic conditions; 2) how they want to learn about medicines; and 3) perceptions of community pharmacist provided counseling. Key findings indicate that children are not frequently present at pharmacies during prescription pick-up; however, children and parents are comfortable with and receptive to pediatric medication counseling by pharmacists. Interactive and educational technologies need to be developed and used by pharmacists to facilitate counseling and educate children about the effective and safe use of medicines.
