APhA 2017 Meeting - CPF Research Forum & More
The city of San Francisco was alive with energy and so was the convention center for APhA 2017. The CPF Board and Staff were also busy making connections, attending sessions, networking, and discussing new grant topic ideas with existing and new applicants among many other things. Joshua Feldmann, PharmD - Evaluation of Community Pharmacist-Managed Transitions of Care on Hospital Readmission Rates

The objective of this practice-based research study was to 1) test the impact of a community pharmacist-delivered care transition intervention on hospital readmissions and 2) describe the problems identified and actions taken by the pharmacists. Same-hospital 30-day readmissions were significantly lower for intervention patients (8.3%) compared to non-intervention patients (21.4%) (p<0.001). Problems identified by the pharmacists included side effects, nonadherence, and cost issues which were resolved using patient education, adherence aids, and prescriber consultations.

The primary purpose of this grant was focused on the engagement of active and influential independent community pharmacists in the NCPDP standards-development process. During this grant cycle, the selected participants have increased their participation in NCPDP's Work Group meetings, and their presence has made an immediate impact. Their perspective and input is critical to NCPDP and its work because the standards that are developed affect the working environment that pharmacists deal with on a daily basis. Active participation is required to ensure that patient safety, which is of paramount importance, is maintained.

Pharmacist Statewide Protocols: Key Elements for Legislative and Regulatory Authority - Report
The Statewide Protocol Workgroup, convened by the National Alliance of State Pharmacy Associations and the National Association of Boards of Pharmacy, has developed policy recommendations for pharmacist statewide protocols. These recommendations include the key elements that should be included in state laws and regulations authorizing the development of statewide protocols – including the components that should be included in the protocol itself.
