Creating a model for successful integration of a community pharmacist into the care transition process
Incorporation of the 4-steps listed in the ‘Starting a Transition of Care Service Toolkit’ led to several ongoing collaborative community pharmacist and community hospital team-based care services.
Addressing barriers to organizational change in NC community pharmacies implementing enhanced services
Utilize the Leading Pharmacy Change interactive website, that was created from this grant, to define stages of change and methods for value stream mapping, and to facilitate implementation of enhanced pharmacy services.

'Tis the Season' to be Mindful of Mental Health First Aid
The holiday season often brings joy, but also unique stressors. The latest issue of the American Pharmacists Association (APhA) Pharmacy Today publication features an article titled 'On the frontlines - Equipping pharmacists with Mental Health First Aid.' Not only does it provide valuable information about Mental Health First Aid training, but it chronicles CPF grant-related activity starting with an initial grant to the National Community Pharmacy Association (NCPA) (Kevin Day, Clarke Bishop, Hannah Fish). It also highlights ongoing activities from the Iowa Pharmacy Association (IPA) (Anthony Pudlo).
NCPA and IPA, among many others, continue to educate and train pharmacists on this very important topic.
