Get The Medications Right (GTMR)
On May 24, 2016, Health2 Resources and Blue Thorn Inc., released a CPF-funded report titled, 'Get the Medications Right' (GTMR) describing a nationwide snapshot of expert pharmacy practices conducting Comprehensive Medication Management (CMM) in ambulatory / community practice settings. The following activities related to this report occurred during October - December, 2016:
Joint Commission of Pharmacy Practitioners (JCPP) - November 29, 2016 -- Terry McInnis, MD shared highlights of the GTMR report with a group of pharmacy profession leaders. Referencing a PowerPoint slide that was adapted from Daniel Buffington, PharmD, MBA, (Clinical Pharmacology Services-CPS), Dr. McInnis began the presentation describing pharmacists' value in the progression of clinical pharmacy service intensity. Patient Advocacy Leaders Summit (PALS) Network Webinar - December 15, 2016. During this webinar, Dr. McInnis targeted her discussion on patient engagement and advocacy. Presentation slides and audio recording links, including audience Q&A, are available in the December CPF Newsletter.
APhA 2017 Meeting - CPF Research Forum
CPF will conduct the 2nd Annual CPF Research Forum at APhA 2017 in San Francisco on March 24, 2017 from 2:30 - 3:30 PM. It will provide an opportunity for select Community Pharmacy Foundation (CPF) grant recipients to showcase their work and dialogue about current and future funding opportunities in community pharmacy. The session is targeted at community pharmacy practitioners and researchers, as well as potential partnering organizations who are interested in learning more about CPF, patient care projects and resources that can be used in practice. Join us for this one-hour session to hear brief grantee presentations followed by an open discussion period.

Elizabeth was selected to present in a special session at the APhA 2017 meeting highlighting her interventions to (1) establish the first Collaborative Practice Agreement (CPA) in Maine providing Medication Management Services in a Primary Care Practice and (2) integrate Fall Risk Prevention Week coordination with local community pharmacies for patients of the Thriving in Place program.

Brian Isetts, RPh, PhD, BCPS, FAPhA - Integrating MTM Provided by Community Pharmacists into a Community-based ACO

This project provides community pharmacists with key steps for building and joining community care teams that will support new Medicare Access and CHIP (Children's Health Insurance Program) Reauthorization Act (MACRA) reimbursement strategies. Although challenges related to implementing new models occurred, the project successes were attributable to the integration of pharmacists credentialed to provide MTM services into new community-based care teams supporting value-based reimbursement.
Rebecca Chater, RPh, MPH - Evaluation of the Impact of Community Pharmacy-Based Medication Synchronization on Patient Adherence

Community pharmacies successfully deployed an appointment-based medication synchronization program, significantly improving chronic medication use. There were 2,262 subjects, ≥18 years of age enrolled across 29 pharmacies located in 19 states. The Proportion of Days Covered (PDC) scores of the enrolled subjects for Star Ratings adherence exceeded the 2016 5-star thresholds in all three categories—diabetes (93.59%), hyperlipidemia (92.68%), and hypertension (91.50%).

A detailed synopsis summarizes the project goals and findings to (1) develop, (2) implement, and (3) evaluate processes and tools for physicians to refer patients to pharmacists for medication management services. In general, physicians expressed needs for patients getting help with managing medications that matched pharmacist MTM services that have been offered and successfully referred patients to pharmacists for those services. The physician referral made a positive impact on patients meeting with the pharmacists to receive the MTM services.
Ana Hincapie, PhD - Comprehensive analysis of Error-prone Medications in Electronic Prescribing

The main goal of this project was to analyze quality related events and errors related to e-prescribing that affect community pharmacist and patients to develop best practice recommendations for improving patient safety. Software developers and vendors have the potential to greatly impact the number of e-prescribing related incidents by adopting three strategies in their systems: (1) use of standardized drug descriptions, (2) use of appropriate prescription quantities, and (3) maintenance of up-to-date prescriber and pharmacy information in the Surescripts directory.
Kenneth Hohmeier, PharmD, RPh - Consumer Perceptions of Point-of-care Testing

The main goal of this project was to identify demographics and willingness-to-pay data of the market niche of consumers who prefer to receive point-of-care test (POCT) services in the community pharmacy using a panel matched to the US population. There was a niche group of consumers who prefer the community pharmacy for POCT services. They are younger, with the largest segment between the ages of 20-34. Compared to a matched sample of the US population, this niche group is willing-to-pay more for POCT services.
