American Pharmacists Association Foundation Pinnacle Awards
Award Ceremony - September 19, 2016 | APhA Terrace | Washington, DC
The Pinnacle Awards, established in 1998 by the APhA Foundation, celebrate significant contributions to the medication use process through increasing patient adherence, reducing drug misadventures, promoting the use of national treatment guidelines, improving patient outcomes or enhancing communication among the members of the healthcare team.
Several CPF Grantees were part of the Category II and III award recipients.

United States Department of Health and Human Services (HHS)
A large team contributed to the success and national improved healthcare results through contributions of leadership, alignment, synergy and hard work to reduce Adverse Drug Events (ADEs) from 2011-2014.
CPF grantees who were part of the HHS team include:
Steven Chen, PharmD, FASHP, FCSHP, FNAP - he has made many outstanding national medication safety and effectiveness contributions as an Associate Professor at the University of Southern California (USC) - School of Pharmacy where he supervises clinical and consultative pharmacy services for 19 safety net community clinics in Southern California. Brian Isetts, PhD, BCPS, FAPhA is a Professor in the Department of Pharmaceutical Care and Health System at the University of Minnesota. Dr. Isetts' field of expertise is in the scholarship of caring, specifically studying the outcomes of medication therapy management services provided in integrated, team-based care settings. Recommendations for Improving Adult Vaccination Rates and Reporting within Community Pharmacy Practice: A White Paper by the PQA Adult Immunization Task Force – According to the new white paper released by PQA, immunizations are an essential component of public health. In recent years, many efforts have been made to increase adult vaccination rates; however, coverage still remains below the national Healthy People 2020 goals. Community pharmacists are poised in an ideal position to help improve adult vaccination rates. This white paper provides best practice recommendations for improving adult vaccination rates and reporting within community pharmacy.
Participants Selected for Part D Enhanced Medication Therapy Management Model – On October, 3, 2016, the Centers for Medicare & Medicaid Services (CMS) Center for Medicare and Medicaid Innovation (Innovation Center) announced the participants in the Part D Enhanced Medication Therapy Management (MTM) model. This Enhanced MTM model offers an opportunity and financial incentives for basic stand-alone Part D Prescription Drug Plans (PDPs) in selected regions to offer innovative MTM programs in lieu of the standard CMS MTM model, aimed at improving the quality of care while also reducing costs.
Bundled Payments for Care Improvement (BPCI) Initiative: General Information – The Bundled Payments for Care Improvement (BPCI) initiative is comprised of four broadly defined models of care, which link payments for the multiple services beneficiaries receive during an episode of care. Under the initiative, organizations enter into payment arrangements that include financial and performance accountability for episodes of care. These models may lead to higher quality and more coordinated care at a lower cost to Medicare.

Congratulations Grantees!
Randy McDonough Named NCPA Independent Pharmacist of the Year
Randy McDonough, RPh, co-owner of three independent community pharmacies in Iowa, has been named the National Community Pharmacists Association (NCPA) 2016 Willard B. Simmons Independent Pharmacist of the Year. The award is sponsored by Upsher-Smith Laboratories, Inc. McDonough received his award during the First General Session at the NCPA 2016 Annual Convention held October 15-19.
The Independent Pharmacist of the Year award recognizes an independent pharmacist for exemplary leadership and commitment to independent pharmacy and to their community. The award is named in honor of someone who exemplified these qualities: Willard B. Simmons, a former executive secretary of NCPA (which was then known as the National Association of Retail Druggists) and a long-time NCPA Foundation trustee.
Randy has successfully completed three CPF funded grants where we often say he 'opens his pharmacy books' and shares the nuances of independent community pharmacy operations.

Allison (Huet) Haas, PharmD | Pharmacist/Owner at Jackson Pharmacy & Wellness Center
This project was implemented at one of the Kaup Pharmacy independent community pharmacies in New Bremen, Ohio. Community pharmacists worked with patients immediately following discharge to reconcile their medications and make recommendations to optimize therapy. The pharmacy packaged their new medication regimen in clear, individual dose adherence packaging. Medications were delivered by a staff driver to the patient's home within 72 hours of discharge. Patients consulted with the pharmacist by videoconference using a computer tablet device. Patients received telephone follow-up shortly before their medication supply was to run out, and additionally as needed on an individual basis.
